For each care request, select the date and start time using the pickers. In the end time field, type the expected end time and any scheduling flexibility (e.g., "8:00 AM— can start as late as 10:00 AM if needed")

Driving Requirements

Is driving required during this job? If yes, a waiver will be signed in the Services Agreement at the end of this form. Let us know whose vehicle will be used (yours or the nanny’s). If the nanny uses her car, a mileage fee will apply.

Water Activities

Are you planning any water activities (pool, beach, etc.)? If yes, a waiver will be signed in the Services Agreement at the end of this form.
